PREVENÇÃO DE LESÕES MUSCULOESQUELÉTICAS EM ATLETAS: UMA REVISÃO
SISTEMÁTICA E META-ANÁLISE DE ENSAIOS CLÍNICOS RANDOMIZADOS

Nas mais diversas modalidades esportivas, a alta prevalência de lesões incapacita e traz custos
elevados para os atletas e sociedade. Embora diferentes estratégias de prevenção de lesões
tenham sido investigadas na literatura, ainda há inconsistências nos achados e lacunas
relacionadas às modalidades esportivas, aos tipos de intervenção e aos desfechos analisados.
Portanto, o objetivo desta revisão sistemática de ensaios clínicos randomizados foi investigar a
eficácia de intervenções para a prevenção de ocorrência de lesões musculoesqueléticas no
esporte e tempo de afastamento, e a aderência as intervenções. Métodos: Realizaram-se
buscas no Medical Literature Analysis and Retrieval System Online (MEDLINE), Cochrane
Library, Excerpta Medica Database (EMBASE), AMED (Allied and Complementary Medicine
Database), CINAHL (Cumulative Index to Nursing and Allied Health Literature), PsycInfo
database, SPORTDiscus e Physiotherapy Evidence Database (PEDro), sem restrições de idioma
ou data, para ensaios clínicos controlados randomizados. A seleção dos ensaios, a extração de
dados e a avaliação do risco de viés foram realizadas independentemente por dois revisores, e
as divergências foram resolvidas por um terceiro revisor. As estimativas de efeito foram
apresentadas como riscos relativos (RR) agrupados com intervalos de confiança (IC) de 95%,
utilizando o método de Mantel-Haenszel. A certeza da evidência foi avaliada utilizando a
abordagem GRADE (Grading of Recommendations Assessment, Development and Evaluation).
Resultados: foram incluídos 104 estudos controlados randomizados investigando os efeitos de
diferentes abordagens para prevenção de lesões musculoesqueléticas (treinamento
neuromuscular, treinamento de força, órteses, dentre outros) em diferentes modalidades
esportivas (futebol, corrida, vôlei, dentre outros), predominantemente esportes coletivos. Em
relação aos desfechos de interesse investigados, 24 estudos avaliaram a incidência de novas
lesões, permitindo a realização de meta-análise. Observou-se redução significativa no risco de
lesões no futebol a favor do treinamento neuromuscular (RR 0,77; IC95% 0,62–0,96) e do
treinamento de força (RR 0,33; IC95% 0,23–0,47), enquanto não houve diferença significativa
para treinamento neuromuscular em esportes gerais (RR 0,91; IC95% 0,60–1,39). Dos 104estudos incluídos, 50 avaliaram tempo de afastamento e 59 avaliaram adesão, entretanto
devido a heterogeneidade, foi realizado síntese descritiva. A certeza da evidência avaliada
pelo GRADE variou de muito baixa para baixa, sendo predominantemente muito baixa.
Conclusão: Programas de prevenção de lesões neuromusculares e baseados no fortalecimento
muscular foram associados à redução da incidência de lesões esportivas, particularmente em
esportes coletivos como o futebol. No entanto, a certeza das evidências foi baixa,
principalmente devido às limitações metodológicas, heterogeneidade e imprecisão. As
evidências que apoiam intervenções passivas isoladas foram limitadas. Embora as estratégias
baseadas em exercícios pareçam promissoras, seu impacto clínico geral permanece incerto.
Ensaios clínicos randomizados de alta qualidade com relato padronizado de desfechos são
necessários para fortalecer a base de evidências.

Abstract: In various sports, the high prevalence of injuries is a significant fact. Although
different injury prevention strategies have been investigated in the literature, there are still
inconsistencies in the findings and gaps related to sports, types of intervention, and the
details developed. Therefore, the objective of this systematic review was to investigate the
efficacy of interventions for the prevention of musculoskeletal injuries in sports and time away
from play, and adherence to these interventions. Methods: Searches were conducted in the
Medical Literature Analysis and Retrieval System Online (MEDLINE), Cochrane Library, Excerpta
Medica Database (EMBASE), AMED (Allied and Complementary Medicine Database), CINAHL
(Cumulative Index to Nursing and Allied Health Literature), PsycInfo database, SPORTDiscus,
and Physiotherapy Evidence Database (PEDro), without language or date restrictions, for
randomized controlled trials. Trial selection, data extraction and risk of bias assessment were
conducted independently by two reviewers, with disagreements resolved by a third reviewer.
Effect estimates were presented as pooled relative risks (RRs) with 95% confidence intervals
(CIs) using the Mantel–Haenszel method. The certainty of the evidence was assessed using the
Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach.
Results: A total of 104 randomised controlled trials were included, investigating the effects of
different approaches for the prevention of musculoskeletal injuries (neuromuscular training,
strength training, orthoses, among others) across various sports modalities (football, running,
volleyball, among others), predominantly team sports. Regarding the outcomes of interest, 24
studies assessed the incidence of new injuries, allowing meta-analysis. A significant reduction
in injury risk in football was observed in favour of neuromuscular training (RR 0.77; 95% CI
0.62–0.96) and strength training (RR 0.33; 95% CI 0.23–0.47), whereas no significant
difference was found for neuromuscular training across general sports (RR 0.91; 95% CI 0.60–
1.39). Of the included studies, 50 reported time-loss outcomes and 59 reported adherence;
however, due to heterogeneity, these outcomes were synthesised descriptively. Certainty of
evidence assessed using the GRADE approach ranged from very low to low, with very low
certainty predominating. Conclusion: Neuromuscular and strength-based injury prevention programmes were associated with reductions in sports injury incidence, particularly in team
sports such as football. However, the certainty of the evidence was low, mainly due to
methodological limitations, heterogeneity and imprecision. Evidence supporting isolated
passive interventions was limited. Although exercise-based strategies appear promising, their
overall clinical impact remains uncertain. High-quality randomised controlled trials with
standardised outcome reporting are needed to strengthen the evidence base.
