Pessoas em hemodiálise frequentemente apresentam fraqueza muscular
inspiratória, associada a prejuízos na qualidade de vida relacionada à saúde (QVRS), na
capacidade de exercício, na mobilidade e no nível de atividade. Entretanto, o papel mediador
da aptidão cardiorrespiratória e da força muscular periférica nessas associações ainda não é
completamente conhecido. Objetivo: Avaliar os efeitos mediadores da aptidão
cardiorrespiratória e da força muscular periférica nas associações entre a força muscular
inspiratória e desfechos relacionados à QVRS, à capacidade de exercício, à mobilidade e ao
nível de atividade em pessoas em hemodiálise. Métodos: Dois estudos transversais foram
conduzidos com pessoas em hemodiálise, avaliadas quanto à força muscular inspiratória, por
meio da mensuração da pressão inspiratória máxima (PImáx); à aptidão cardiorrespiratória,
pelo Incremental Shuttle Walk Test; à QVRS, pelos componentes físico (CFS), mental (CMS) e
geral sumarizados (CGS) do questionário Kidney Disease Quality of Life (estudo 1); à f orça
muscular periférica, pela mensuração da força de preensão palmar (FPP); à ca pacidade de
exercício, pelo teste de sentar e levantar de 60 segundos (TSL-60); à mobilidade, pelo teste de
velocidade de marcha de 4 m (VM); e o nível de atividade, pelo Perfil de Atividade Humana
(PAH) (estudo 2). Análises de mediação simples foram realizadas considerando a PImáx como
variável independente, os desfechos funcionais e de QVRS como variáveis dependentes, e a
aptidão cardiorrespiratória (estudo 1) e a FPP (estudo 2) como variáveis mediadoras.
Resultados: No estudo 1, a aptidão cardiorrespiratória exerceu efeito mediador parcial na
associação entre a PImáx e CFS, com proporção de mediação de 32,5%. Não houve efeito
mediador na relação entre PImáx e CGS, nem associação entre PImáx e CMS. NO estudo 2, a FPP apresentou efeito mediador parcial significativo nas associações entre a PImáx e TSL-60,
VM e PAH, com proporções de mediação de 36,4%, 37,8% e 41,7%, respectivamente.
Conclusão: Em pessoas em hemodiálise, a associação entre a força muscular inspirató ria e a
capacidade de exercício, a mobilidade e o nível de atividade é parcialmente mediada pela FPP,
enquanto a relação com os aspectos físicos da QVRS é parcialmente mediada pela aptidão
cardiorrespiratória.

Abstract: Introduction: People undergoing hemodialysis frequently present with inspiratory
muscle weakness, associated with impairments in health-related quality of life (HRQoL),
exercise capacity, mobility, and activity level. However, the mediating role of cardiorespiratory
fitness and peripheral muscle strength in these associations is not yet fully understood.
Objective: To evaluate the mediating effects of cardiorespiratory fitness and peripheral muscle
strength on the associations between inspiratory muscle strength and outcomes related to
HRQoL, exercise capacity, mobility, and activity level in people undergoing hemodialysis.
Methods: Two cross-sectional studies were conducted with people undergoing hemodialysis,
assessed for inspiratory muscle strength, through the measurement of maximum inspiratory
pressure (MIP); cardiorespiratory fitness, using the Incremental Shuttle Walk Test; and
HRQoL, using the physical (CFS), mental (CMS), and general summarized (CGS) components
of the Kidney Disease Quality of Life questionnaire (study 1); Peripheral muscle strength was
measured using handgrip strength (HGS); exercise capacity was assessed using the 60-second
sit-to-stand test (60-second sit-to-stand test); mobility was assessed using the 4-meter gait
speed test (GST); and activity level was assessed using the Human Activity Profile (HAP)
(study 2). Simple mediation analyses were performed considering MIP as the independent
variable, functional and HRQoL outcomes as dependent variables, and cardiorespiratory fitness
(study 1) and HGS (study 2) as mediating variables. Results: In study 1, cardiorespiratory
fitness partially mediated the association between MIP and CFS, with a mediating proportion
of 32.5%. There was no mediating effect on the relationship between MIP and CGS, nor an
association between MIP and CMS. In study 2, FPP showed a significant partial mediating
effect in the associations between PImáx and TSL-60, VM, and PAH, with mediation
proportions of 36.4%, 37.8% and 41.7%, respectively. Conclusion: In hemodialysis patie nts,
the association between inspiratory muscle strength and exercise capacity, mobility, and activity level is partially mediated by FPP, while the relationship with the physical aspects of
HRQoL is partially mediated by cardiorespiratory fitness.
