The effects of exercise-based programs on the activity and participation of people with neuromuscular diseases: a systematic review

    Objetivo: Investigar quais programas de reabilitação são capazes de modificar a Atividade e Participação de pessoas com Doenças Neuromusculares (DNM). Data search: EMBASE, BVS/Lilacs, PEDro, CINAHL/EBSCO and MEDLINE were searched in June/2021. It was last updated in September/2022. Methods: Randomized controlled trials investigating any rehabilitation or non-pharmacological therapy for people with NMD with an outcome encompassing the Activity and Participation of the International Classification of Functioning, disability and health (ICF) components were included. The results were synthesized according to the Core Sets for NMD. The methological quality and level of the evidence were assessed using Physiotherapy Evidence Database (PEDro) criteria and Grading of Recommendations Assessment, Development, and Evaluation (GRADE), respectively. This systematic review followed the PRISMA 2020 guideline. Results: Dos 1728 estudos identificados, 12 foram incluídos nesta revisão com qualidade metodológica entre regular e boa. O exercício aeróbio de intensidade leve a moderada foi a intervenção mais estudada. A mobilidade foi avaliada em todos os estudos incluídos tendo como intervenções o exercício aeróbico, fisioterapia aquática, terapia comportamental e musical. A qualidade da evidência da reabilitação para a Mobilidade (12/12), Autocuidado (5/12), Comunicação (2/12) e Participação Social (6/12) de pessoas com DNM é muito baixa. Conclusion: A variabilidade dos tipos de DNM e o reduzido número amostral dos estudos incluídos torna muito baixa a evidência das intervenções voltadas para a Atividade e Participação de pessoas com DNM. O exercício aeróbio de intensidade leve a moderada parece melhorar a mobilidade, autocuidado e participação social de pessoas com DNM, principalmente as de progressão lenta.

Light to moderate intensity aerobic exercise was the most studied intervention. Mobility was evaluated in all included studies with interventions such as aerobic exercise, aquatic physiotherapy, behavioral and music therapy. The quality of rehabilitation evidence for Mobility (12/12), Self-Care (5/12), Communication (2/12) and Social Participation (6/12) of people with NMD is very low. Conclusion: The variability of the types of NMD and the small sample size of the included studies makes the evidence of interventions aimed at the Activity and Participation of people with NMD very low. Light to moderate intensity aerobic exercise seems to improve mobility, self-care and social participation in people with NMD, especially those with slow progression.
