Introdução: A doença de Chagas (DC) é uma infecção causada pelo protozoário Trypanosoma
cruzi. A doença se apresenta clinicamente em duas fases distintas, a aguda e a crônica, sendo
que a última pode revelar-se nas formas indeterminada, cardíaca, digestiva ou cardiodigestiva.
A cardiomiopatia chagásica (CCh), forma cardíaca da infecção, é a mais grave das
manifestações clínicas e a mais comum, sendo responsável pela alta taxa de morbimortalidade.
Nessa fase, a fadiga e dispneia aos esforços físicos habituais estão entre os achados mais
comuns, tido como um marcador bem definido de pior prognóstico, contribuindo para a redução
da capacidade funcional. O teste de caminhada de seis minutos (TC6’) é um método útil na
avaliação da capacidade funcional de pacientes cardiopatas e apesar de ser amplamente
utilizado sua função prognóstica na CCh permanece desconhecida. Objetivo: Determinar a
associação entre a distância do TC6’ e o pico do consumo de oxigênio (VO2pico) e verificar a
acurácia na predição de eventos cardiovasculares adversos em pacientes com CCh. Métodos:
Estudo prospectivo, realizado com setenta pacientes com CCh (43 do sexo masculino, 48±7
anos), que foram avaliados por ecocardiografia, teste de esforço máximo e TC6’. Os pacientes
foram acompanhados através de entrevistas telefônicas por 41,4 meses e o desfecho foi definido
como morte cardíaca, eventos cerebrovasculares ou transplante cardíaco. Resultados: No final
do período de acompanhamento, 20 pacientes (29%) apresentaram eventos adversos. A
distância percorrida no TC6’ estava associada ao VO2pico (r=0,552, p=0,001). A distância de
491m foi o ponto de corte ótimo na predição de eventos cardiovasculares. Não houve diferença
na frequência dos eventos cardiovasculares adversos (p<0,133) entre os grupos com baixa
(≤491m) e alta capacidade funcional (>491m). Na análise univariada de Cox, a menor fração
de ejeção de ventrículo esquerdo (FEVE) (p=0,002), o maior diâmetro diastólico final do
ventrículo esquerdo (p=0,011) e o TC6’ inferior a 491m (p=0,133) foram associados a pior
evolução. Na análise multivariada, apenas a FEVE permaneceu como um preditor independente
de pior prognóstico (HR 0,94; IC 95%: 0,90 a 0,98, p=0,002). Conclusão: O TC6’ se mostrou
uma ferramenta válida na avaliação da capacidade funcional em pacientes com CCh. Não foi
encontrada evidência que sustente o valor prognóstico do TC6’ em pacientes com CCh.

The disease presents itself clinically in two distinct phases, the acute and the chronic, and the
latter can be revealed in indeterminate, cardiac, digestive or cardiodigestive forms. Chagas
cardiomyopathy (CCh), the cardiac form of the infection, is the most severe of the clinical
manifestations and the most common, being responsible for the high morbidity and mortality
rate. In this phase, fatigue and dyspnea on habitual physical exertion are among the most
common findings, considered a well-defined marker of worse prognosis, contributing to the
reduction of functional capacity. The six-minute walk test (6MWT) is a useful method for
assessing the functional capacity of patients with heart disease and, despite being widely used,
its prognostic role in CHD remains unknown. Chagas disease (CD) is an infection caused by

malignant arrhythmias, thromboembolism, stroke, progressive myocardial deterioration, and

functional capacity. The six-minute walk test (6MWT) is a useful method in assessing the
functional capacity of patients with heart disease. Despite being widely used, prognostic test
function in patients with ChC remains unknown. Objective: To determine the association
between the 6MWT distance and peak oxygen consumption (VO2peak) and verify the accuracy
in predicting adverse cardiovascular events in patients with CCh. Methods: Prospective study,
carried out with seventy patients with ChCh (43 males, 48±7 years), who were evaluated by
echocardiography, maximal effort test and 6MWT. Patients were followed through telephone
interviews for 41.4 months and the outcome was defined as cardiac death, cerebrovascular
events or heart transplantation. Results: At the end of the follow-up period, 20 patients (29%)
experienced adverse events. The distance covered in the 6MWT was associated with VO2peak
(r=0.552, p=0.001). The distance of 491m was the optimal cutoff point for predicting
cardiovascular events. There was no difference in the frequency of adverse cardiovascular
events (p<0.133) between the groups with low (≤491m) and high functional capacity (>491m).
In the univariate Cox analysis, the lowest left ventricular ejection fraction (LVEF) (p=0.002),

the highest left ventricular end-diastolic diameter (p=0.011) and the 6MWT less than 491m
(p=0.133) were associated the worst evolution. In the multivariate analysis, only LVEF
remained as an independent predictor of worse prognosis (HR 0.94; 95% CI: 0.90 to 0.98,
p=0.002). Conclusion: The 6MWT proved to be a valid tool in the assessment of functional
capacity in patients with CCh. No evidence was found to support the prognostic value of the
6MWT in patients with CCh.
